Overview
The Event Captain Report is a form in FormAssembly that captains complete after an event. It is filled in with information from Delphi, the Statler Hotel's Salesforce: you pick the event's BEO Number first, and the form uses it to look up the booking and fill in parts of the report for you. The completed form creates an Event Captain Report record in Delphi.
Log in with your NetID. The form is behind Cornell single sign-on because it is filled in with data from Delphi, so you sign in with your NetID and password.
Start a report
- Go to https://johnson.tfaforms.net/115 and log in with your NetID and password.
- Click the BEO Number field and select the BEO for this report. Type the start of the BEO Number to filter the list, or scroll through it.
- Click Submit. This opens the next part of the Event Captain Report (a separate form) with the event's details already filled in from the booking.
Which BEOs appear in the list
The BEO Number list comes from the booking's documents in Delphi. It only shows BEO documents whose event date is today or yesterday, so a report has to be started within a day of the event.
The Event Captain form type is selected for you. The Culinary Team form is not currently an option.
