Event Captain Report - User Documentation

The 3 Event Captain Report forms are in FormAssembly, meaning it can use existing Salesforce data to prepopulate parts of the form and automatically create the form input (Event Captain Report object in Salesforce).  The forms are behind SSO via SAML, so require a netID login to complete the form.  This form replaces a Qualtrics form.

Report statuses

StatusMeaning
In ProgressThe captain has started the report but not finished it.
Ready for ReviewSubmitted and waiting for review.
ReturnedSent back because it needs more information. Nothing is emailed.
ApprovedReviewed and complete. The PDF has been emailed to the banquet captains.

After a captain submits an Event Captain Report, it is reviewed in Delphi, the Statler Hotel's Salesforce, and then either Approved or Returned . Approving a report emails its PDF to the banquet captains and records who approved it and when.
