You can make a formal inquiry regarding specific Facilities and Campus Services' information or data. These requests can range from simple to complex demands that can be provided in .xlsx, .csv, or .PDF format. Please note submitting this form is not a commitment to satisfy the request.
Knowledge Base Documentation Request
General information, instructions, troubleshooting guidance, knowledge base articles, and training are available in the FCS Ticket Toolbox Knowledge Base. Service delivery managers are responsible for the accuracy of this information. Customers can suggest updates, additions, or changes here.
System Access Request
Managers can use this form to request updates to an individual's access to systems such as: Kuali Financial System (KFS), Kuali Data Warehouse, KDW local security, and KDW Act As Proxy.
Systems Support
This service exists to address computer software, hardware, and telecommunication needs for Facilities and Campus Services (FCS) at a technical and functional level. The needs include but are not limited to; Access, Billing, Integration, Networking, Software Disruption, and more.
TDX Improvement Request
Customer feedback informs FCS’s continual service improvement efforts. End users and business customers may submit an improvement via ticket. Feedback is reviewed and escalated to the appropriate service owner, service delivery manager, business process owner, or other responsible FCS resource.
