By submitting this form, I acknowledge that I have read this form in its entirety and that the information I have provided is true and correct to the best of my knowledge. I authorize the Cornell Law School’s Financial Aid Office to process my federal loans. I permit Cornell University to release loan disbursement information to me electronically. Please note that in order to receive a federal loan for the first time, the Electronic Master Promissory Note must be signed and online Entrance Counseling completed before loans will disburse.

For PLUS Loan Borrowers: I consent to the U.S. Department of Education, Cornell University, and its agent’s permission to obtain a report of my credit record and to use the information from that report in determining whether to make a Direct Grad PLUS loan to me. I understand that I will be notified in writing of the results of the credit check with respect to my loan application.

I understand that if I am a Title IV federal aid recipient, this aid will be credited to my account to cover all educational expenses. Such expenses may include, but are not limited to, tuition, fees, room and board, and items such as extra course fees and the student health insurance plan. If I elect to have federal aid credits limited to tuition, fees, and room and board, I will notify the Law School’s Financial Aid Office, in writing, to this effect.
